Referral Forms for GPs

General Mental Health / Alcohol & Other Drugs

For mild to moderate mental health issues prepare a Mental Health Treatment Plan 2715 or 2717 (template below). If you have not completed Level 1 Mental Health Training, you can claim using 2700 or 2701.  Our preferred method of receiving referrals is through Medical Director, choose 'FOCUS SCDGP General' .  Alternatively fax your referral letter or a FOCUS referral form along with the Mental Health Treatment Plan to 5482 4959. Let your patient know that we will be in touch with them once the referral is received.

For Alcohol & Other Drug counselling choose 'FOCUS SCDGP General' in Medical Director to refer electronically OR fax your referral letter or a FOCUS referral form to 5482 4959. (NOTE: A treatment plan is not required for Alcohol & Other Drug referrals.) Let your patient know that we will be in touch with them once the referral is received.

Suicide / Self Harm

For patients at risk of suicide or self harm send your referral by:

Medical Director -  'FOCUS SCDGP Suicide'   OR  Fax -  5443 7107and please call us on 5443 7100 to ensure we are in receipt of your referral. Let your patient know that we will be in touch with them once the referral is received.

Suicide / Self Harm referrals do not require a 2715/2717; however, the more information we receive the better we can help your patient.
